Contest Site Request Form
Please include this form with your
District VI registration and fee on or before October 20, 2011. Every attempt will be made to place your group(s) at the site(s)
you have requested. There may be a need to rearrange those sites once the
numbers are submitted. You may be asked to move sites and/or dates in
order to keep the numbers balanced. Contest Assignments will be made by
November 11, 2011 and will be listed on the District VI web page at IGSMA.org.

Membership
fee is due on or before Monday, September 19, 2011.
(Please note that
late payments will be assessed a $15.00 late fee.)
District Membership fee is $50 per junior high campus.
Also note that State Membership is a separate fee. Checks should be
made payable to: IGSMA

State Forms: These forms are separate from the
District. You must be a member at the State Level in order to be a member
at the District Level.
